Patient Check In

A New Way to Check-In for Your Appointment

At Nevada Eye Physicians, we realize your time is valuable. In an effort to streamline your appointment, we have installed self-check-in kiosks right in the lobby at all 4 of our locations. This highly-secure, electronic process is beneficial for several reasons:

  • Eliminates repetitive forms
  • Eliminates the need to wait in line at the front desk
  • Reduces your time in the waiting room
  • Increases efficiency for our staff
  • Improves the overall patient experience

All you need to bring is your driver’s license and health insurance card. You may also need a credit card if your appointment will require payment. To start, you’ll simply place your driver’s license into a well-marked slot on the machine and it will automatically input your information. If you do not have a driver’s license and/or insurance card, you will have the option to input your information manually. There will be other questions to answer during the check-in process.

Mobile Pre-Check options help expedite your appointment by allowing you to complete your paperwork prior to your appointment. After scheduling an appointment with us you will receive a registration notification where you will be able to access your patient forms and complete them prior to your appointment. Your paperwork will be digitally sent to us and be ready for the day of your appointment.
